By signing this document, you will waive certain rights including the right to sue.
I allow my child to participate willingly in any activities while attending Vaughan Sportsplex II (Which can include any of the following: sports and/or any other activities) at Vaughan Sportsplex II, Sportsplex II, Vaughan Sportsplex Inc, Sportsplex Ltd, Vaughan Sportsplex 2 and GTA Sportsplex Ltd., and agree to the following release: I hereby voluntarily release, forever discharge, and agree to indemnify and hold harmless Vaughan Sportsplex II, Sportsplex II, Vaughan Sportsplex Inc, Sportsplex Ltd, Vaughan Sportsplex 2 and GTA Sportsplex Ltd & their directors, officers, employees, agents and representatives (all of whom are hereinafter collectively referred to as "Vaughan Sportsplex") and to waive any and all claims, demands, or causes of action, that I have or may have in the future against, Vaughan Sportsplex and to release, Vaughan Sportsplex from any and all liability for any loss, damage, expense or injury including death that I may suffer or that my family, heirs, assigns, personal representatives and estate may suffer as a result of my participating in Vaughan Sportsplex sports, or activities. DUE TO ANY CAUSE WHATSOEVER, INCLUDING NEGLIGENCE, BREACH OF CONTRACT, OR BREACH OF ANY STATUTORY OR OTHER DUTY OF CARE, INCLUDING ANY DUTY OF CARE OWED UNDER THE OCCUPIERS' LIABILITY ACT, R.S.O. 1990, c . 0. 2 ON THE PART OF VAUGHAN SPORTSPLEX AND FURTHER INCLUDING THE FAILURE ON THE PART OF VAUGHAN SPORTSPLEX TO SAFEGUARD OR PROTECT ME FROM THE RISKS, DANGERS AND HAZARDS OF VAUGHAN SPORTSPLEX, OR ACTIVITIES (HEREINAFTER REFERRED TO AS "CLAIMS").

This Agreement shall be governed by and interpreted in accordance with the laws of the Province of Ontario and any litigation involving the parties to this Agreement shall be brought within Ontario.

I understand and accept the risks associated with Vaughan Sportsplex and all the activities, including but not limited to the possibility of physical or emotional injury, paralysis, death, and property damage. I understand that this agreement applies not only to sports activities, but also all other equipment, and all activities and games at Vaughan Sportsplex and that they may give incomplete warnings or instructions. They cannot always watch all participants. The equipment being used might malfunction. I VOLUNTARILY ACCEPT AND ASSUME ALL OF THE RISKS ASSOCIATED WITH ALL ACTIVITIES AT VAUGHAN SPORTSPLEX AND I CHOOSE TO PARTICIPATE DESPITE THE RISKS.

I have read and understood this agreement and am aware that by signing this agreement I am waiving certain legal rights which I or my heirs, next of kin, executors, administrators, and assigns may have against Vaughan Sportsplex.
